New State Appointment Request

Use this form to request to add a new state to one of your *existing* carrier appointments. You must be ready to sell with the carrier to request a new state. You can verify your ready-to-sell status on the Spark Platform.

Please have your licenses ready for upload. If you are an upline agency principal, you may submit on behalf of your downlines.

Note: if you are *only* adding a new state for Wellcare, you may skip the form and add it directly by following the instructions at this link.
